COURSE DESCRIPTION:
As the home health population shifts toward higher medical complexity, home health physical and occupational therapists must look beyond basic functional assessments to prevent unnecessary hospital readmissions. This course provides home health clinicians with advanced, evidence-based clinical reasoning strategies to safely manage complex geriatric patients with multi-morbidities.
Participants will learn how the OASIS-E2 updates shift from a mindset of administrative compliance to active predictive screening for adverse events and will come away with tools to improve ability to identify and guide real-time therapeutic interventions.
Beyond physiological monitoring, this course addresses the direct impact of chronic hypoxia and low cardiac output on patient health literacy and working memory. Attendees will learn to audit their clinical jargon and implement evidence-based techniques to drive patient safety and chronic disease adherence.
Clinicians will leave with a practical framework to confidently modify interventions based on objective clinical data, maximizing patient independence while ensuring medical stability.
LEARNING OBJECTIVES:
- Apply OASIS-E2 data elements and current AHA/AGS clinical measures to assess medical risk for geriatric patients with cardiopulmonary instability.
- Evaluate the impact of health literacy on patient safety and justify the clinical necessity for interdisciplinary intervention to mitigate the risk of negative consequences such as medication errors and falls.
- Perform a systematic multisystem medical clearance (Cardiac, Respiratory, and Metabolic) before implementing interventions, especially those of higher intensity.
- Relate symptomatic orthostatic hypotension based on Mean Arterial Pressure (MAP) calculations with patients’ safe participation in functional activities, including mobility, self-care, household management, or recreation.
AUDIENCE:
This advanced course is designed for OTs and PTs working with adult populations in home care, outpatient and rehab settings.
Robyn Ligotti, PT, DPT, RAC-CT, CDP, QCP is the owner of Rehab Resource Solutions, LLC Consulting and is a licensed DPT with over 35 years of experience. The scope of her practice has encompassed many settings including: skilled nursing facilities, home health, outpatient rehab, wellness and prevention and inpatient rehab, but always with the older adult population. She has managed multi-disciplinary teams in skilled nursing facilities, home health and outpatient rehabilitation. Her Certifications include: Dementia Practitioner, Resident Assessment Coordinator and Quality Assurance Performance Improvement Certification. She is dedicated in her belief that the key to achieving strong outcomes is establishing a team of professionals who use critical thinking evidence-based skills in day to day practice. She has provided education to rehab and nursing teams and other disciplines throughout her career on safe patient handling, understanding Medicare and rehab specific treatments and approaches to care. Dr. Ligotti graduated with her B.S. in P.T. from Russell Sage College and received her Doctor of P.T. from Sage Graduate School.
