COURSE DESCRIPTON:
Frenectomy surgeries are often done without adequate pre and post op care. According to the research the most effective way of managing tethered oral tissue is with pre and post operative therapy. The goals of a pre op program include increasing range of motion, decreasing fascial tension, insuring the client can tolerate input and maximizing functional goals for feeding, swallowing and speech. Pre operative programs are performed by professionals including IBCLCs, speech-language pathologists, occupational therapists, physical therapists, craniosacral therapists, chiropractic physicians and osteopaths. The goals of post operative therapy include active wound management to maintain the integrity of the wound and mitigate scar tissue, and neuromuscular neuromuscular re-education techniques that support functional goals for pre and post op care re-education to remediate functional challenges.
There is a lack of consensus regarding how to best perform post op wound care and ambiguity about scope of practice and training for individuals providing care. Families are often asked to do “stretches” multiple times a day without adequate knowledge and training. Research, to date, on the efficacy of active wound care is limited.
Neuromuscular re-education is focused on functional goals for addressing oral motor skill development to support feeding, speech and resting posture. These goals are guided by the oral sensory motor milestones that infants and toddlers achieve within the first three years of life. These skills serve as a pre-requisite for feeding, swallowing and speech across the lifespan. Modalities for the 0-3 population may include lactation support, pre-feeding, therapeutic feeding and body work.
This three hour presentation will use lecture, video demonstrations and interactive practical experiences to demonstrate neuromuscular re-education techniques that support functional goals for pre and post op care.
LEARNING OBJECTIVES:
- Identify at least two benefits of pre/post op therapy for clients with tethered oral tissues.
- Differentiate between active wound management and neuromuscular re-education post frenectomy surgeries.
- Choose an appropriate pre op treatment option for addressing oral motor skill development based upon a functional assessment.
- Choose an appropriate post op treatment option for addressing oral motor skill development based upon a functional assessment.
REQUIREMENTS:
Please have either “Veggie Stix”, “Veggie Straws”, or thin pretzel sticks available for a practicum during the Pre and Post op session.
AUDIENCE:
Birth to three therapists including OTs, PTs, SLPs, and Special Educators.

Lori L. Overland, M.S., CCC-SLP, C/NDT, CLC, FOM is a licensed speech and language pathologist with her neurodevelopmental certification and certificate as a lactation counselor as well as having a fellowship in orofacial myology. Lori specializes in oral sensory-motor/feeding therapy, oral placement and orofacial myofunctional disorders in children. Lori’s private practice focuses on infants, toddlers and preschool children with oral sensory-motor/feeding disorders. Lori consults with children from all over the world providing evaluations, re-evaluations, program plans and week long, family-centered feeding therapy programs. Lori also provides consults to local school districts and Birth-to-Three organizations. Her goal in addressing feeding and speech challenges is to improve the quality of life for both the children she serves and their families.
Lori has teamed up with Robyn Merkel-Walsh M.A., CCC-SLP/COM to provide mentoring services through Mouths in Motion Mentoring. In addition to her private practice, Alphabet Soup, Lori is a member of the TalkTools® speakers’ bureau. Lori has lectured on sensory-motor/feeding disorders around the United States and internationally. Lori was accepted, multiple times, to present poster and live presentations on a sensory-motor approach to feeding, self-limited diets, tethered oral tissue, and complementary feeding at the annual convention of the American Speech, Language and Hearing Association. Lori is a member of ASHA, CSHA, LISHA, NYSSLHA, the IAOM, ICAP, a board member of the oral motor institute and a Breathe Institute Ambassador. Lori holds degrees from Hofstra University and Adelphia University.
